THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, HOW YOU CAN GET ACCESS TO THIS INFORMATION, AND YOUR RIGHTS REGARDING YOUR HEALTH INFORMATION. PLEASE REVIEW IT CAREFULLY.
You have certain rights regarding your health information. These include the right to:
Access your records. You may request an electronic or paper copy of your health information. In most circumstances, a copy or summary will be provided within 30 days of your request. A reasonable, cost-based fee may apply.
Request corrections. You may ask Daylight Grief to correct health information that you believe is incorrect or incomplete. If your request cannot be granted, you will be informed of the reason in writing.
Request confidential communications. You may ask to be contacted in a specific way or at a particular address. Reasonable requests will be accommodated.
Request restrictions. You may ask that certain health information not be used or shared for treatment, payment, or health care operations. Daylight Grief is not always required to agree to these requests.
If you pay for a service completely out of pocket, you may request that information about that service not be shared with your health insurer for payment or health care operations, unless disclosure is required by law.
Receive an accounting of disclosures. You may request information about certain disclosures of your health information made during the six years preceding your request.
Receive a copy of this notice. You may request a paper copy of this Notice of Privacy Practices at any time, even if you received it electronically.
Choose someone to act on your behalf. If another person has legal authority to act on your behalf, that individual may exercise your privacy rights as permitted by law.
File a complaint. If you believe your privacy rights have been violated, you may contact Daylight Grief or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. Daylight Grief will not retaliate against you for filing a complaint.
In certain circumstances, you may tell Daylight Grief your preferences regarding the use or disclosure of your health information, including sharing information with family members, friends, or others involved in your care.
Certain uses or disclosures require your written authorization. You may revoke an authorization in writing as permitted by law.
Daylight Grief does not sell your personal health information.
Daylight Grief may use or disclose protected health information as permitted or required by law, including:
Treatment. Information may be used or shared with other health professionals involved in your care when appropriate and permitted by law.
Payment. Information may be used or disclosed to obtain payment for services, including when submitting claims to health plans.
Health care operations. Information may be used as necessary to operate the practice, manage services, and perform other permitted health care operations.
Information may also be disclosed when permitted or required by law, including certain circumstances involving public health and safety, health oversight activities, legal proceedings, law enforcement, workers' compensation, and preventing or reducing a serious threat to health or safety.
Because psychotherapy and mental-health records may receive additional protections under federal or New Jersey law, Daylight Grief will comply with applicable laws that provide greater confidentiality protections.
Psychotherapy notes receive additional protection under federal privacy law. Except for certain limited circumstances permitted by law, Daylight Grief will obtain your written authorization before using or disclosing psychotherapy notes.
Federal law provides additional protections for certain substance use disorder treatment records subject to 42 CFR Part 2.
When Daylight Grief maintains information that is subject to these protections, that information will be used and disclosed in accordance with applicable federal law.

Daylight Grief may change the terms of this notice. Changes may apply to health information already maintained by the practice as well as information received in the future.
